How Do I Choose a Breast Implant?

During the consultation process, Dr. Silberblatt talks with you to establish what you hope to achieve through surgery. With this information, he can guide you toward the implant type, size, and shape that will best achieve your goals. We offer both saline and silicone implants in a variety of shapes and sizes. While Dr. Silberblatt may recommend certain options based upon your goals, the decision is ultimately up to you. Dr. Silberblatt’s role is to provide a better understanding of what the options offer your unique body structure so you can choose the implants you will be most pleased with. What Is a Keller Funnel®?

Keller Funnel Certification LogoThe Keller Funnel is a tool that speeds up the actual process of placing the implant and reduces surgery time. It also allows the implants to be inserted with less overall trauma to the implant shell and allows Dr. Silberblatt to make a smaller incision. Most critically, it allows the implant to avoid all contact with the skin, which is important because residual bacteria on the skin or in the breast tissue can increase the risk of capsular contracture (scar tissue forming around the implant, giving it a hardened feel). With the Keller Funnel, the risk of developing capsular contracture is somewhat reduced. The Keller Funnel also allows Dr. Silberblatt to put less stress on the implant during insertion and minimizes residual scarring because of the smaller incision required. Using the Keller Funnel is another way Dr. Silberblatt employs state-of-the-art instruments and techniques in breast augmentation procedures to give you the best and most predictable results. He was the first to start using the funnel here in Roanoke.

Should the breast implant be placed over or under the muscle?

Placement of the implant over or under the pectoral muscle depends on skin thickness and breast tissue development or shape. If the skin is very thin, placement under the muscle is preferable. Implant shape and folds are less likely to show through the muscle’s added thickness. Patients should recognize, however, that the bottom outer half of the implant will not be covered by muscle because of the anatomy of the muscle. This means that wrinkles and folds are more likely to be seen or felt in the bottom half of the breast mound. Implants placed under the muscle are more likely to migrate from their ideal position, and surgery is more painful because of trauma to the muscle. However, many patients and surgeons think submuscular implants look more natural. If skin or breast tissue needs to be stretched out, then placement over the muscle is preferable.

If too much skin is present, and the breast is droopy, then a breast lift is necessary to achieve a good contour. What Breast Augmentation Incision Is Best?

Breast Augmentation IncisionsThe process of inserting breast implants requires making a small incision under the breast, around the areola, in the armpit, or at the belly button. Dr. Silberblatt most commonly makes the incision under the breast because, at this location, the incision is only 1.5 inches long and heals well. This location also gives Dr. Silberblatt direct access to the pocket and allows him to easily see everything necessary. This location is also far enough away from the nipple and breast tissue to decrease the chance that breast duct bacteria might find its way to the surface of the implant.

The incision around the areola is also popular because the scar usually heals well and is discreet. However, the scar is sometimes visible as a white line adjacent to the pigmented areolar tissue, and this incision location can increase the chance that bacteria will contaminate the implant surface. The armpit incision is well-hidden but hurts more and has a higher chance of resulting in implants that are out of position. Dr. Silberblatt does not offer the incision around the belly button because that approach can only be used with saline implants, and the results he has seen presented by surgeons at national meetings are aesthetically inferior to the results using other incisions.

How do I choose the right size implant?

You need to take several factors into account when determining the right implant size, including your lifestyle, body contour, and desired aesthetic outcome. If you have an active lifestyle, larger implants can make it more difficult to perform certain physical activities, such as yoga, sports, and running. Additionally, implant size not only indicates the amount of volume being added to each breast pocket, but it also affects your body contour. It is important to consider how your breasts’ projection will affect your overall silhouette.

Another consideration is your ultimate cosmetic goal. Because larger implant sizes also have a wider diameter, patients with thin breast skin may have an increased chance of implant rippling when compared to smaller implant sizes. In turn, larger implants that do not correspond to your body dimensions may create an appearance that makes your surgical augmentation more apparent. The choice of a subtle enhancement or noticeable transformation, however, is ultimately at your discretion.

During your sizing appointment, you can try on a variety of implant sizes with a special surgical garment to get a feel for which implants provide the right fit. You are also encouraged to bring pictures from magazines or websites to provide a clearer idea of what outcome you envision. While Dr. Silberblatt can provide his professional recommendations, deciding on the most satisfactory implant size is ultimately up to you.

Do breast implants need replacing?

Although both silicone and saline implants have the potential to last for decades, they are not designed to last a lifetime. You may need to replace your implants if there are complications. At Aesthetic Surgery of Virginia, we aim to provide comprehensive care to our patients long after their treatment ends, which is why any implants used at our practice come with an extended warranty program.

For your safety and convenience, all silicone and saline-filled implants are automatically enrolled in the warranty program. Through this program, the product manufacturer guarantees 10 years of financial support and lifetime product replacement, among other benefits. You also have the option of upgrading, for a small charge, to a program with longer coverage and higher reimbursement.

Breast Augmentation Timeline

The typical sequence of visits for a breast augmentation procedure at Aesthetic Surgery of Virginia is as follows:

  • A free pre-consult with one of our staff, if desired
  • The initial consultation and evaluation ($50)
  • A second, free consultation to answer any additional questions, if desired
  • A separate visit for implant sizing unless it is combined with one of the initial visits
  • A history and physical exam no more than one week prior to surgery
  • Surgery
  • Wound check the day after surgery
  • Suture removal one week after surgery
  • Post-operative checks at 3 weeks, 6 weeks and 3 months after surgery
  • Post-operative photographs (available to the patient) at 3 months

How long does it take to recover from breast augmentation?

Recovery varies after surgery. Patients usually require 1 to 2 weeks off work. You may carefully begin active exercise at about 3 weeks after surgery, and all restrictions are usually lifted 6 weeks after surgery. Your post-surgery plan is developed according to your needs and desires. We discuss relevant risks and benefits at your initial consultation and provide information packets listing these considerations. The vast majority of patients are thrilled with their results.
