FTM Transgender Breast Surgery
The surgical reduction of breast tissue is often the first transgender operation female to male patients receive when undergoing a sex change. This operation removes and decreases the visibility of breast tissue in order to achieve a more masculine-looking chest. It can be life-changing and extremely rewarding for the right patient.
One recent patient works in a building supply store and spent much of his life trying to hide the fact that he was a genetic female. He has always been interested in things that are generally associated with males such as maintenance work, automobiles and construction. Since his surgery, he has been able to go about his job and hobbies without concern that someone might notice his breasts. He told us, “You have no idea how much this has changed my life!”
FTM Breast Surgery Procedure
At my office, the specific transgender breast surgery technique I utilize depends on the amount of breast tissue present in the patient. For the individual with a limited amount of breast tissue, the operation I typically perform combines liposuction and open breast tissue removal under sedation or general anesthesia. Once the targeted fat tissues in the breast have been removed, a small amount of tissue generally remains around the areola. This breast tissue cannot be aspirated like fat and needs to be excised. I make a semicircular incision below the areola through which the remaining breast tissue is extracted for the most natural-looking outcome.
If the patient has a moderate amount of breast tissue, the nipple may need to be transferred to a new site higher on the chest, and I perform a free nipple graft to restore a natural appearance. In this instance, I typically use a infra-mammary incision. If a substantial amount of tissue is present in the breasts, I often perform an inverted T incision and free nipple graft. Regardless of the technique I utilize, liposuction is generally employed to remove fatty tissue in the surrounding area.

Recovery from FTM Breast Surgery
Recovery from female to male transgender breast surgery will depend upon the patient’s natural healing processes. A compression garment is placed after surgery to facilitate healing, and drains are typically removed the second day following the operation. Most patients can expect to return to work after about five days; however, those whose jobs require strenuous activity should expect to rest about three weeks before returning to work.
Female to male breast surgery is an irreversible procedure. Prior to performing this operation, I require a signed letter of consent from the patient’s counselor. For this letter, the counselor must specify that he or she knows the patient, that he or she has been seeing him for an extended period of time, and that the patient is fit to undergo this irreversible surgery.
