Otoplasty is an operation that pins back ears that stick out excessively. The ear itself is normally shaped except for a very deep bowl and a fold near the edge that may be shallow or missing. The operation that I have found to best correct this problem involves incisions behind the ear through which I release the bowl of the ear and rotate it towards the head. I also rasp the cartilage near the edge of the ear under the skin to weaken it and allow me to sew it from behind to create a new, natural looking fold. No incisions are made on the visible side of the ear. Some plastic surgeons like to cut the cartilage to create the missing fold but I feel this results in a sharp outline that looks abnormal. A bulky, protective dressing is put on after surgery and remains on for one week. Afterwards, a headband is used for another five weeks.
This operation is most commonly done on children at about the age of five, just before they enter school. However, there are many adults who never had an opportunity to have this corrected as a child and live with it by wearing their hair long and over their ears. Many women with prominent ears would like to wear their hair up but won’t because they are self conscious about it. The operation can be done under local anesthesia or with sedation, according to patient preference. This operation has among the highest satisfaction rates of any that I do.
